Who buys first, and where can we expand?

The first planned cohort comes from acknowledged healthcare interest. Multi-site nursing home operators offer a further route to recurring facility work.

Medicare- or Medicaid-certified nursing homes in the August 2026 CMS snapshot used in our market work.

Certified beds, from our sum of the same file.

Dated market analysis, October 1, 2026, using CMS Nursing Home Provider Information, August 2026. Source dataset. These are facility and bed counts, not revenue forecasts.

One relationship can span many facilities.

Our August 2026 ownership analysis groups 10,110 homes under 610 operators with at least two homes.

Those homes account for about 71% of beds in the file. A common ownership identifier does not establish a shared purchasing decision.

Our computation from CMS ownership data: 1,106,891 of 1,567,504 beds. The analysis excludes 10 homes without a bed count. Method and source snapshot available in diligence.

Build beyond the initial workflow.

  1. Convert acknowledged institutional interest into scoped workforce pilots.
  2. Expand to adjacent facilities, including multi-site nursing home operators.
  3. Add clinician types with their own requirements and source coverage.
  4. Earn recurring clinician use through a useful career record, opportunities and professional relationships.
  5. Extend workforce operations into the wider facility workplace where customers demonstrate demand.
  6. Investigate workforce intelligence for additional buyers only where the use case, data rights and economics are established.

Planned expansion. Each step requires its own deployment scope, permissions and operating evidence.

Choose the first customer by the work we can deliver.

A facility count describes market structure. A serviceable first customer also needs an accountable buyer, a permitted workflow and a deployment the team can support.

The public market sizing uses certified nursing homes because the source is inspectable and aligns with the planned certified-bed pricing unit. The acknowledged letters span a broader set of healthcare employers. Those are different evidence sets: the letter portfolio does not establish nursing-home market share, and the nursing-home count does not price every organization in the letter portfolio.

Initial qualification should establish the workflow and role family, the owner of the problem, source availability, the decision authority and a measurable cost or delay. An organization without certified beds needs its own agreed commercial unit before a paid proposal can use the pricing model.

What makes a prospective pilot actionable?
Demand
A sponsor names a recurring operational problem and the result that would justify continuing. General interest in AI is not enough to define the scope.
Authority
The actual budget holder, workflow owners and data/security reviewers are identified. A supportive contact may not have authority to sign for all sites.
Delivery
The selected source and destination routes, staff availability and operating requirements can support the agreed path. Unresolved dependencies remain part of qualification.
Measurement
Operations and finance agree what will be counted, the observation period and the total effort budget. The baseline can be inspected before value is claimed.

The sale, the deployment and the workflow are different units.

An operator may sponsor a relationship spanning several facilities. Each activated site still has its own people, evidence routes and operating requirements. A workflow cohort is narrower again: the cases admitted to the measured scope. Investor reporting should distinguish all three rather than convert one operator conversation into several customer deployments.

Commercial relationship
Which legal counterparty can enter the agreement, which budget funds it, and which sites the agreement actually covers?
Activated facility
Which site has completed its requirements and is operating the authorized scope? Group affiliation alone does not establish activation.
Measured cohort
Which roles and cases are included, over which dates, with which sources and completion rules? The denominator must stay visible.

Multi-site relationships could lower repeated commercial and deployment effort. That advantage must appear in time to activation, support burden and results at subsequent sites. A shared ownership identifier is a research lead; it is not proof of centralized procurement or identical policy.

Build the revenue case from eligible beds and actual adoption.

Bottom-up model · planned

Recurring revenue = contracted eligible beds × agreed monthly rate × active months.

At the facility level, the agreed production rate is constrained by the planned signed-savings cap. At the portfolio level, the model must include conversion, time to activation, contraction and retention. It should not apply a list price to every bed in the source file and present the result as a revenue forecast.

The 100-bed illustration on Business model shows the commercial unit without assuming adoption. The wider opportunity can be modeled with explicit facility counts, bed counts and product eligibility, then tested against sales capacity and deployment capacity. The slower of those two constrains the near-term plan.

Each expansion earns its own market definition.

New role types bring different requirements and verification routes. New facility types can require a different purchasing unit. Whole-practice operations introduce new departmental owners and data boundaries. A clinician community needs repeat participation; an intelligence offering needs a specific permitted decision and a buyer willing to pay for it.

We treat these as connected expansion paths rather than adding unrelated markets into one headline number. The strategic attraction is shared utility: work creates a useful record, the record supports a continuing professional relationship, and authorized operating history can improve the next task. The vision package lays out the sequence and the evidence that would justify each next investment.

This chapter combines the dated CMS analysis with proposed qualification and modeling methods. It does not assert current market penetration, conversion rates or a validated sales cycle.
